- Victorian Fatalities Release - December 2009

 A 24 year old man died at Rutherglen after being hit by the attachment on a hose belonging to a concrete pump while a 21 year old who suffered an electric shock at Stawell later died in hospital.

- Operator Pinned Against Safety Fence - 240609

An operator was recently pinned between a parent/hard roll and a safety fence when the roll moved forward unexpectedly. The operator suffered bruising and soreness but was able to return to work on alternative duties.

- Heidelberg Speedmaster - Printing Press Guarding - 260509

A 49yo man's arm was dragged into a printing press while he was operating the 1st unit of a 4 colour Heidelberg Speedmaster VP press.  His arm was drawn into the press between the plate cylinder and the inking roller.  Emergency services were unable to free the injured person's arm and doctors decided to amputate his arm at the scene of the indcident.

- Paper Machine - Fatal Accident - 280409

A 45yo man was fatally injured while attempting to remove broke paper from a drying cylinder.  The deceased was attempting to remove paper from a drying cylinder and became trapped between the drying cylinder and a dryer fabric roll.

- Skid Steer Loader - 160409

An employee in the UK was killed whilst operating a JCB 'Robot' 170 skid-steer loader.  Following further investigations, the HSE have released this information bulletin.

- PM Breast Roll Cable Failure—030409

Whilst a paper machine breast roll was being raised into the run position, the front cable sheared, followed by the rear, causing the breast roll to fall from its suspended pivotal arc position.  This failure resulted in the breast roll colliding with the machine frame making significant impact which caused damage to the immediate area.  No personal injuries were sustained.

- Felt Section Doctor Blade Removal—110309

A worker was injured whilst extracting a felt section return doctor blade.  The worker was using a vice grip tool to pull the blade to the front of the machine and wearing a leather glove to provide support and manoeuverability required for extraction.  The blade cut through the glove and sliced the left hand ring finger.

- Paper Machine Reel Spool Incident—020209

A paper mill employee suffered a broken jaw and crush injuries to the mouth and cheek bone when his head came into contact with a reel spool while in the process of preparing a small reel of paper for storage.  The combined weight of the reel spool and paper involved was approx. 13.5 tonnes.

- Fall Into Sawing Equipment in Pulp Mill-170109

A 49yo man was killed when he apparently fell into sawing equipment at the Duino mill in Italy.  The emergency stop system of the saw failed for reasons not yet clear.

- IPUK-130109

Following a paper break, an employee sustained a serious injury, which resulted in later amputation of the lower left arm at the elbow.  The injured person was tidying up paper tails at the reel up area.  It appears he became entangled with a heavy gram sheet, which resulted in his arm being pulled across the shell and into the nip joint.
